How Pilates Can Relieve Back Pain

Is back pain keeping you from doing the things you love? It’s a common complaint, but fortunately, there are several things you can do to manage back-related discomfort. However, one of those approaches might surprise you! Pilates for back pain is an effective way to build strength and reduce discomfort–which is why we offer it at Mobility Project Physical Therapy as a complement to our physical therapy programs.
Understanding Pilates: A Simple Yet Powerful Approach to Movement
Pilates is a low-impact exercise method that focuses on improving core strength, posture, mobility, and body awareness. Developed in the early 20th century by Joseph Pilates, this method was designed to help people move with greater control and balance.
Unlike high-impact workouts that can strain your joints and muscles, Pilates uses controlled movements and deep breathing to help stabilize the spine and support healthy alignment.
Why Pilates Works for Back Pain
One of the biggest reasons Pilates is so effective for back pain is its focus on the core—the group of muscles that wrap and support your spine, including your obliques, back muscles, hip muscles, and deep internal muscles.
When these deep muscles are weak or not working properly, your lower back takes on more strain. Pilates strengthens those muscles in a safe and controlled way, which can ease tension and prevent injury.
Here’s how Pilates helps support a healthier back:
- Improves spinal alignment by reducing unnecessary pressure on discs and joints
- Encourages balanced muscle use by avoiding overuse of the lower back
- Enhances stability and coordination, making everyday movements safer and easier
Regular Pilates training is particularly helpful for people with chronic back pain. It can help them return to work faster, need fewer pain medications, and enjoy a better quality of life.
Key Pilates Exercises That Support Back Health
You don’t need a reformer machine to benefit from Pilates. Mat-based Pilates—performed on the floor with just a mat, which is what we offer at Mobility Project Physical Therapy—is highly effective for back care. Below are a few common exercises we incorporate into our program.
Dead Bug
- This core-stabilizing exercise builds deep abdominal strength while keeping the spine neutral. It encourages coordinated movement between arms and legs which helps reduce compensation from the lower back and reinforces core control for safer movement during daily tasks.
Knee Folds (Single Leg Lifts)
- This is a simple but effective way to activate the deep core without straining the spine. We use it to train the abdominals and pelvic stabilizers. This also teaches control during small, precise leg movements and is safe for those with lower back sensitivity.
Swimming (Prone Arm/Leg Lifts)
- This exercise targets spinal stabilizers and back extensors without creating pressure on joints. It helps to strengthen the muscles along the spine (especially the erector spinae) and helps improve posture and muscular endurance. It also encourages coordinated limb movement while maintaining core engagement.
Tabletop Toe Taps
- This exercise teaches lumbopelvic control and reinforces core stability with minimal spine movement. It engages abdominal muscles without overusing the hip flexors while supporting pelvic alignment and reducing strain on the back.
Wall Roll Down
- We use this to help enhance full-spine articulation exercise that helps improve mobility and core control. This exercise increases flexibility in the spine by encouraging segmental movement awareness and helps to decompress the spine gently with gravity support.
Our team will work with you to select the right Pilates exercises for your needs. We’ll look for movements that activate the right muscles without stressing your joints.
When done under the guidance of our physical therapists, Pilates for back pain significantly reduces discomfort and improves movement.
